Our services cater to individuals who:
Require crutches, canes, wheelchairs, or human assistance to leave home
Have dementia/Alzheimer’s or memory impairment
Are unable to use stair handrails due to upper extremity loss
Experience limited ambulation due to pain and/or weakness
Suffer from heart or respiratory diseases
Did You Know?
None of the following outings necessarily disqualify someone from home health services:
Attending religious services
Occasional visits to the barber or salon
Leaving for certain healthcare services and treatments
Using state-certified, licensed, or accredited adult day care services
Participating in family reunions, funerals, graduations, or weddings
Medicare Coverage:
If you have Medicare, you qualify for our services. Medicare allows temporary increases in outings, such as visits from out-of-town family, without affecting home health eligibility. Care planners consider a patient’s ability to leave home over their entire care plan and permit short-term exceptions.
